AccidentSurance

Declaration for AccidentSurance
I, on behalf of each of myself, my spouse and my child/children (if any), hereby declare as follows:
1. I (Insured)/We (Insured, insured spouse and insured child/children) declare that the statements and particulars given in this application are to the best of my/our knowledge and belief, true and complete, and that this application will form the basis of my/our contract with HSBC Insurance (Asia) Limited.
2. I/We further declare that:
(i) I am/We are in good health and free from physical impairment or deformity.
(ii) I and my insured spouse are both under 60 years of age and the insured child/children is/are six months of age or over but under 18 years of age or is a/are full-time student(s) at a school, college or university under 23 years of age.
(iii) I am/We are Hong Kong Special Administrative Region residents.
(iv) I/We have never been denied personal accident insurance.
(v) I/We do not have any in-force AccidentSurance policy with HSBC Insurance (Asia) Limited that was effected after 27 October 2002, and have not had any AccidentSurance policy cancelled during the last six months.
3. I/We understand that insured person(s) who engage in work or activities listed in the Hazardous Activities List, will receive no benefits under the policy for Accidental Injury, Death or Medical Expenses which result from or are caused directly or indirectly by an Accident whilst carrying out such work or activity.
4. I/We have read and understood the Terms and Conditions and the Policy Wording of this AccidentSurance and agree to be bound by the same.
Terms and Conditions for AccidentSurance
1. Eligibility (age limits)

Applicant's family members are:

  1. legally married spouse under 60 years of age
  2. all legally dependent unmarried children, including step children and legally adopted children, who are six months of age or over but under 18 years of age or full-time students at a school, college or university under 23 years of age
2. Benefits Payable
(i)Spouse:
Same as the Insured's
(ii) Children:
Cash Benefit for Accidental Injury or Death: 20% of the Insured
Medical expenses: 20% of the Insured's (Daily limit same as the Insured's)
Chinese bonesetter expenses: 20% of the Insured's (Daily limit same as the Insured's)
3. Premium Payment Method
Premiums may be paid by direct debit to a designated credit card account.
4. Renewal Agreement
AccidentSurance will be renewed upon payment of premium when due, otherwise it will be cancelled automatically.
5. Refund of Premium
(i)For monthly payment policy
This policy is subject to minimum payment of two months' premium in advance which is not refundable upon termination unless the policy is cancelled within 15 days of the receipt of the policy (i.e. from the date of application).
(ii)For annual payment policy
This policy is subject to the payment of the annual premium in advance. All premium paid will at all times be subject to the Premium Refund Rules upon the termination of AccidentSurance except where AccidentSurance is cancelled within 15 days of the receipt of the policy (i.e. from the date of application), where a full refund will be made.

Fire Insurance

Declaration for Fire Insurance
1. I (Applicant) have read and understood the Personal Information Collection Statement.
2. I declare that the statements and particulars given in this application are to the best of my knowledge and belief, true and complete, and this application will form the basis of my contract with HSBC Insurance (Asia) Limited. I further declare that:
(i)My insured property is built of bricks, stone and concrete and roofed with concrete.
(ii)My insured property is less than 40 years of age.
(iii)My insured property is not used for commercial purposes and is located in a permanent building in Hong Kong SAR.
(iv)I am/will be the legal owner of this insured property by the Policy Commencement Date.
(v)I have never had any fire insurance applications rejected or accepted on special terms by any insurance company.
(vi)I do not have any Fire Insurance policy of my insured property with HSBC Insurance (Asia) Limited cancelled during the last six months.
(vii)I understand that, in respect my choice of sum insured, this policy is subject to Average clause (refer to clause 11 of the policy) and Reinstatement Value Insurance (refer to clause A19 of the policy) in the event of under-insurance.

3. Premium Payment Method

4. Renewal Agreement
Renewal letter will be sent to your last known correspondence address about 45 days prior to your policy's annual expiry date. The policy will be automatically renewed for another year on successfully debiting the annual premium from your designated direct debit account.
5. Refund of Premium
This policy is subject to minimum payment of HK$400 or following short period refund rate, whichever is higher.

FirstCare Medical Insurance

Terms and Conditions for FirstCare
1. Eligibility
The following family members of the applicant who are eligible for coverage:
  • Legally married spouse under 75 years of age (or under 65 years of age if applying for Company Top-up Plan or Outpatient benefit)
  • All legally dependent unmarried children, including step children and legally adopted children, who are aged 15 days to 17 years (inclusive) or in full-time education at a school, college or university under 23 years of age
2. Premium Payment
Premium must be paid by direct debit from a designated credit card account.
3. Renewal Arrangement
  • Renewal letter will be sent to your last known correspondence address about 45 days prior to your policy expiry. Renewal and subsequent annual premiums will be collected from your designated credit card account. Upon receipt of your renewal premium, we will issue new medical card(s) and schedule of benefits to you.
  • Renewal is arranged automatically and is guaranteed for life. The premium payable upon policy renewal and the terms of any renewal may not be the same as for the expiring policy and will be determined by HSBC Insurance (Asia) Limited at its absolute discretion.
4. Change of plan
Change of plan type is not allowed during the policy period. For change of plan type upon policy renewal, you should submit your request for change at least one month before policy expiry. Any change if approved will only be effective upon policy renewal.
5. Refund of premium
This policy is subject to the payment of the annual premium in advance which is subject to premium refund rule of the policy unless the policy is cancelled within 15 days of the receipt of the policy.

Golfer's Insurance Package

Declaration for Golfer's Insurance Package
1. I (Insured)/We (Insured and insured's spouse) have read and understood the Personal Information Collection Statement.
2. I/We understand that this application will form the basis of my/our contract with HSBC Insurance (Asia) Limited.
3. I/We declare that the statements made in or in connection with this application are true and complete and that I/we have not withheld any material facts which are likely to affect acceptance or assessment of the insurance cover requested. I/We understand that in case of doubt as to whether HSBC Insurance (Asia) Limited should be informed of certain facts, I/we should call HSBC Insurance Service Hotline on (852) 2583 8000. Failure to disclose any material facts referred to above may mean that the Policy will not provide the Insured Person(s) with the required coverage, or may invalidate the Policy altogether.

I/We further declare that

(i)I am /We are both aged between 18 to 65.
(ii)I am/We are amateur golfer(s).
(iii)I am/We are residents of the Hong Kong Special Administrative Region.
(iv)I/We have never had any of my/our applications for personal insurance rejected or have had any of such insurance policies cancelled by any insurance companies.
(v)I/We have never made any claim relating to golfer's equipment, personal effects, hole-in one expenses, personal liability or personal accident under any insurance in the last 3 years.
4. I/We understand and agree that no refund of premium will be made once the insurance is effected.
5. I/We understand that HSBC Insurance (Asia) Limited will send an "invitation to renew" to us regarding renewal of the Policy in the future and my/our application for renewal of the Policy is subject to the approval of HSBC Insurance (Asia) Limited.
6. I/We have read and understand the Terms and Conditions and the Policy Wording of this Golfer's Insurance Package and agree to be bound by the same.
Terms and Conditions for Golfer's Insurance Package
1. Customers should refer to the Terms and Conditions set out below and the Policy Wording of this Golfer's Insurance Package.

    (iv)I understand that this insurance is for the proposed helper(s) who is/are lawfully engaged for domestic duties only.

    1. Eligibility
    Applicant must be 18 (by last birthday) or over.
    2. Premium Payment Method
    Premium can be paid by direct debit to designated credit card account.
    3. Renewal Agreement

    Declaration for HomeSurance
    1. I, on behalf of each of myself and the joint applicant (if any), declare that the statements and particulars given in this application are to the best of my/our knowledge and belief, true and complete, and that this application will form the basis of my/our contract with HSBC Insurance (Asia) Limited.

    I/We further declare that:
    (i)My/Our home is built of brick, stone and concrete and roofed with concrete. The gross floor area stated in this application includes balconies, terraces, forecourt, backyard and/or roof of the Home.
    (ii)I/We have never had any of my/our Home, Personal Belongings, Domestic Helper, Golfer insurance and/or Loss of Rent insurance applications rejected and have never had any such insurance policies cancelled by any insurance companies.
    (iii)My/Our home is in a permanent residential building which is already constructed and less than 40 years of age, or will be constructed by the Policy Effective Date.
    (iv)I/We am/are or will be by the Policy Effective Date, the legal owner/s or the tenant/s of this home.
    (v)I/We do not have any in-force HomeSurance policy with HSBC Insurance (Asia) Limited for this home and have not had any HomeSurance policy for this home cancelled during the last six months.
    2. I/We have read and understood the Terms and Conditions and the Policy Wording of this HomeSurance and agree to be bound by the same.
    Terms and Conditions for HomeSurance
    1. HomeSurance is specially designed solely for domestic use in residential building, house, apartment or flat being situated in the Hong Kong Special Administrative Region (Hong Kong SAR). All structure of the home must be under legal construction.
    2. Premium Payment Method
    Premiums may be paid by direct debit to a designated credit card account.
    3. Basis of Coverage
    This insurance coverage for Household Content and Worldwide All Risks cover is based on 'New For Old'; that is, repairing the damaged property or replacing the property if stolen or beyond repair with a new article of the same kind which is of similar but not better quality.
    4. The successful applicant and his/her family members (including spouse, children, parents and other relatives permanently living with you in the home) must comply with all statutory obligations and take all reasonable steps to prevent loss, damage or injury; and maintain the home in efficient condition and good repair.
    5. The successful applicant must advise us of any change in his/her occupation of the premises, any permanent move of the home or circumstance which would increase the possibility of loss or damage. Premiums may be adjusted accordingly.
    6. Renewal Agreement
    HomeSurance will be renewed upon payment of premium when due, otherwise it will be cancelled automatically.

    2. I/We further declare that:
    (i)I/We have not had, during the last four years, any illness, injury, ailment or condition requiring in-patient treatment or consultation with a specialist.
    (ii)I/We do not have any foreseeable need for treatment or consultation with any medical practitioner.
    (iii)I am, and the insured spouse (if any) is, under 60 years of age and the insured child/children is/are between six months and 21 years old inclusive or is a/are a full-time student(s) at a school, college or university under 23 years of age.
    (iv)I am/We are residents of the Hong Kong Special Administrative Region.
    (v)I/We have never been denied personal accident or medical or hospital cash insurance.
    (vi)I/We do not have any in-force HospitalSurance or Refundable HospitalSurance policy with HSBC Insurance (Asia) Limited, and have not had any HospitalSurance policy cancelled during the last six months.
    3. I/We acknowledge that benefits are not payable under the HospitalSurance scheme for treatment arising from any existing illnesses, injuries or other conditions.
    4. I/We have read and understood the Terms and Conditions and the Policy Wording of this HospitalSurance and agree to be bound by the same.
    Terms and Conditions for HospitalSurance
    1. Eligibility (age limits)

    Declaration for Refundable HospitalSurance
    I, on behalf of myself (as Policyholder and/or Insured Person as applicable), and my spouse/my child (each as Insured Person and as applicable), hereby declare and acknowledge as follows:
    1. The statements and particulars given in this application are, to the best of my/our knowledge and belief, true and complete and that this application will form the basis of my/our contract with HSBC Insurance (Asia) Limited.
    2. I further declare that:
    (i)The Insured Person(s) did not have, during the last four years, any illness, injury, ailment or condition requiring in-patient treatment or consultation with a specialist and the Insured Person has no foreseeable need for treatment or for consulting any medical practitioner.
    (ii)The Insured Person(s) is/are resident(s) of the Hong Kong Special Administrative Region. I and my spouse (if applicable) are, under 60 years of age, and my child(ren) (if applicable) is / are aged between 6 months and 17 years inclusive or is / are in full-time education at a school, college or university under 23 years of age.
    (iii)The Insured Person(s) has / have never been denied, postponed or accepted with restricted benefits or additional conditions in personal accident or medical or hospital cash insurance.
    (iv) The Insured Person(s) does / do not have any in-force HospitalSurance or Refundable HospitalSurance Policy with HSBC Insurance (Asia) Limited.
    3. Benefits are not payable under the Refundable HospitalSurance plan for treatment arising from any pre-existing illnesses, injuries or other conditions.
    Terms and Conditions for Refundable HospitalSurance
    1. Eligibility
    The following family members of the applicant who are residents in the Hong Kong Special Administrative Region are eligible for coverage:
    -Legally married spouse under 60 years of age
    -All legally dependent unmarried children, including step children and legally adopted children, who are aged 6 months to 17 years (inclusive) or in full-time education at a school, college or university under 23 years of age
    2. Premium Payment
    Premium must be paid by direct debit from a designated credit card account. Frequency of payment (whether on monthly or annual basis) is determined at the time of application.
    3. Renewal Arrangement
    -The policy will be renewed for another 10 years for Insured Person(s) under the age of 60 at policy expiry. Renewal letter will be sent to your last known correspondence address about 45 days prior to your policy expiry. Renewal and subsequent premiums will be collected from your designated direct debit account based on the premium payment mode you selected (i.e. monthly payment or annual payment).
    -The policy renewal of this plan is subject to the insurance company's underwriting decision and is not guaranteed. The premium payable upon policy renewal and the terms of any renewal may not be the same as those of the expiring policy and will be determined by HSBC Insurance (Asia) Limited at its absolute discretion.
    4. Change of plan
    Change of plan type is not allowed during the 10-year policy period. For change of plan type upon policy renewal, you should submit your request for change at least one month before policy expiry. Any change if approved will only be effective upon policy renewal.

    TravelSurance

    Declaration for TravelSurance
    1. I hereby declare that I have been duly authorised by each of the persons covered under this application including guardian(s) of the child(ren) mentioned in this application form (together, the "Insured Persons" and each an "Insured Person") to apply for TravelSurance and to make the following declarations for and on his/her/their behalf. I also hereby declare that each of the Insured Persons has agreed to the information under this application including under these Declarations, and that it is a condition precedent to obtaining coverage for each such person that such Insured Person has agreed to all such information. (This declaration is not applicable to an applicant applying for his/her own insurance only.)
    2. I, and on behalf of each of the Insured Person(s), hereby apply for TravelSurance, deemed and accepted to constitute separate insurance in respect of each such Insured Person, and declare that the statements and particulars given in this application are to the best of the knowledge and belief of each of the Insured Person(s), true and complete and that this application will form the basis of the contract of the Insured Person(s) with HSBC Insurance (Asia) Limited. I/Each of the Insured Person(s) understand(s) and agree(s) that no insurance will be effected until the application is approved and the premium is received by HSBC Insurance (Asia) Limited.
    3. I, and on behalf of the Insured Person(s), confirm that (i) I/all the Insured Person(s) are 85 years of age or younger (applicable to Single Trip TravelSurance) / 75 years of age or younger (applicable to MultiTrip TravelSurance); (ii) all children are no younger than six months and no older than 16 years of age; and (iii) all persons described in (i) and (ii) above are residents of the Hong Kong Special Administrative Region and have never been denied travel insurance.
    4. I, and on behalf of the Insured Person(s), hereby acknowledge and warrant that: I will not/None of the Insured Person(s) will be travelling contrary to the advice of any medical practitioner or travelling in order to receive medical treatment. I do not know/None of the Insured Person(s) knows of any condition, cause or circumstance existing that may necessitate the cancellation or curtailment of the planned journey.
    5. I and each of the Insured Persons have read and understood the Terms and Conditions and the Policy Wording of this TravelSurance and agree to be bound by the same.
    Terms and Conditions for TravelSurance
    1. TravelSurance is specially designed for individuals, family members, relatives and friends who are travelling on a journey that originates from the Hong Kong SAR only.
    2. Period of Insurance
    (i)For Single Trip TravelSurance, duration of the period should not be longer than 180 days.
    (ii)For MultiTrip TravelSurance, the coverage will cease 60 days from the beginning of each trip.
    3. Eligibility (age limits)

    (i)Six months to 85 years (inclusive) for Single Trip TravelSurance applications. Six months to 75 years (inclusive) for MultiTrip TravelSurance applications. MultiTrip TravelSurance is renewable to the age of 85.
    (ii)Adult means persons aged between 17 to 85.
    (iii)Children aged between 12 to 16 who are not travelling with their parents, may apply for coverage upon payment of the adult premium, but only child benefits shall apply.
    (iv)Children aged 11 and below must be accompanied by an Insured Adult.
    4. Premium Payment Method
    Premiums may only be paid by direct debit to designated credit card account
    5. Extension of Period of Insurance
    To extend the period of insurance (applicable only to Single Trip TravelSurance) after the policy is issued, please call 2583 8000 or visit our branches for arrangement before the expiry of policy. The extension will be applicable to all the insured persons under the policy. The minimum additional premium is HK$30.

    7. Application for the China Medical Card
    For internet applications, if any of the Insured Persons under "Self & Spouse", "Self & Children" and "Self, Spouse & Children" plans are to be covered by the China Medical Card, all Insured Persons under that policy must apply for an individual card. If not all the Insured Persons require China Medical Card, please call 2583 8000 or visit our branches for arrangement.
    8. Cancellation
    Single Trip TravelSurance
    Once the application is approved, the policy or the coverage of any insured person cannot be cancelled. Should an Applicant choose to cancel his/her application, no premium will be refunded.

    MultiTrip TravelSurance
    Once the application is approved, the insurance can be cancelled during the Period of Insurance by giving notice in writing to HSBC Insurance (Asia) Limited. Such cancellation shall be effective upon the notice being received by HSBC Insurance (Asia) Limited. The premium for the Insured Period shall be calculated based on the short period premium table provided that the refunded premium does not exceed 50% of the annual premium.

    China Medical Card
    Once the application is approved, the China Medical Card option may be cancelled by giving notice in writing to HSBC Insurance (Asia) Limited during the Period of Insurance or by canceling the MultiTrip TravelSurance policy. Such cancellation shall be effective upon the notice being received by HSBC Insurance (Asia) Limited or on the cancellation date of the MultiTrip TravelSurance policy. No refund of premium will be made once the insurance is effected.

    Terms and Conditions for Refundable Protection Plan (Major Illness)
    1. Eligibility
    1.1To be eligible for Refundable Protection Plan (Major Illness) you must be:
    i. age 19-50 (age next birthday) at the policy effective date;
    ii. Hong Kong Identity Card holder; and
    iii. resident in Hong Kong.
    1.2You can only insure your own life.
    1.3You can only be covered under one Refundable Protection Plan (Major Illness) Policy.
    1.4This application must be completed and submitted in the Hong Kong Special Administrative Region.
    2. The beneficiary of the policy will be defaulted to your estate, but you can change it by completing and returning the form "Request for Policy Change - Non Financial" to HBSC Life (International) Limited at 18/F, Tower 1, HSBC Centre, 1 Sham Mong Road, Kowloon, Hong Kong
    3. Premium
    3.1 Premium may only be paid by direct debit from your designated HSBC credit card account or your HSBC bank account.
    3.2You have the right to cancel your policy within the cooling-off period (21 days from the date of this application, 14 days from the date of issue of the Policy or 5 days from the date of delivery of the Policy/a Notice informing you/your representative about the availability of the Policy and the expiry date of the cooling-off period, whichever is later).
    Upon cancellation, you will receive a full refund of any paid premium. To cancel your Policy, you are required to give written notice which must be signed by you and received directly by HSBC Life (International) Limited at 18/F, Tower 1, HSBC Centre, 1 Sham Mong Road, Kowloon, Hong Kong.
    4. The insurance applied for herein shall take effect immediately provided that the initial 2 months premium or the first year premium, whichever is applicable, has been paid in full and you are in good health at the time of payment of the first premium.
    5. The insurance applied for herein shall be made available subject to the Policy Wording and these Terms and Conditions. In the event of any conflict arising in respect of these Terms and Conditions and the Policy Wording issued, the Policy Wording will be considered determinative.
    6. Replacement Declaration

    This is an IMPORTANT PART of the Code of Practice for the Life Insurance Replacement and the Minimum Requirements as specified by the Insurance Authority under the Insurance Companies Ordinance, but does not form part of the application/proposal.

    You confirmed that you have not replaced# in the past 12 months or do not intend to replace# in the next 12 months any or a substantial part of your existing life insurance policy(ies) with this application/proposal.

    You realize if

    1. this application/proposal has replaced any or a substantial part of your existing life insurance policy(ies) in the past 12 months; or
    2. your current intention is to replace any or a substantial part of your existing life insurance policy(ies) within the next 12 months by this application/ proposal.

    You may jeopardise your future right of redress if you find later that you have been disadvantaged because of such replacement.

    You hereby authorize HSBC Life (International) Limited to give the Insurance Agents Registration Board, the Hong Kong Confederation of Insurance Brokers, the Professional Insurance Brokers Association Limited, the Insurance Authority ("IA"), the Hong Kong Federation of Insurers, the insurer(s) of the life insurance policy(ies) that is/are being or has/have been replaced (if applicable) or other parties, as required for proper administration/implementation/execution of the Code of Practice for Life Insurance Replacement and the Minimum Requirements for insurance brokers as specified by the IA under the Insurance Companies Ordinance, a copy of this Replacement Declaration and any related records or information.

    Note:
    #Any transaction involving the purchase of life insurance is construed as a Replacement if (i) any existing life insurance policy(ies) or a substantial part of the sum insured of its/their basic life coverage has been/have been/will be terminated or (ii) a substantial part of the guaranteed cash value of the existing life insurance policy(ies) was reduced/will be reduced including where a policy loan was/will be taken out against a substantial part of the guaranteed cash value. If requested, you agree to provide the details of any physician, hospital, clinic, insurance company or other individual organisation or government office that has medical records or medical knowledge of yours and you hereby authorise any such physician, hospital, clinic, insurance company or other organisation or government office that has such records or knowledge to disclose to HSBC Life (International) Limited or its representative any information relevant to this application. A photocopy of this authorisation shall be as valid as the original.
    9. You authorise The Hongkong and Shanghai Banking Corporation Limited to share with HSBC Life (International) Limited your Hong Kong Identity Card copy where necessary for this application, and agree to send a photocopy of your Hong Kong Identity Card to HSBC Life (International) Limited if required.

    iii. resident in Hong Kong.
